Advance care planning helps you prepare for a time when you may not be able to speak for yourself. It is more than paperwork. It starts with conversations about your values, beliefs, and what matters most. These conversations help you choose a strong healthcare agent, make informed decisions, and reduce stress for loved ones during medical emergencies or serious illness. 


In this presentation, we explore the whole framework of advance care planning in simple, practical terms. We will cover: 

  • How to identify your values and wishes before a crisis

  • How to choose a healthcare agent who can speak on your behalf

  • Filling out advance directive forms, including the Five Wishes document

  • When each form is used, who completes them, and how they guide loved ones and medical teams, including other advance directive forms like POLST, DNR, and more

  • How to talk with loved ones and care teams about your wishes

  • How advance care planning supports autonomy, comfort, and peace of mind

You will leave with a clear understanding of the tools available, how to complete key documents, and how to start essential conversations at home. This session is suitable for adults of all ages, caregivers, family members, and anyone wanting to make thoughtful choices about their future care.
